1. Roster *

Every term DCJS produces a list of members that includes the information above. This allows members to contact each other more easily. This roster will be available to all members in electronic form or on paper for those who request it. It will not be made available to anyone outside DCJS. It will not appear on the publicly accessible pages of our website, and members agree not to use this list for anything other than DCJS business. I agree to the inclusion of my name in the list of members for the above stated purposes unless I have opted out by checking the following statement:

2. Photos *

DCJS is a choral ensemble that regularly performs at public venues and may be photographed, recorded, and videotaped both with and without the advance knowledge of the Director and the members of the choir. These photographs, recordings and/or videos may appear in various forms of print, sound, digital, and internet media. As a member of this choir, I agree to the use of my likeness and voice in image and sound without restriction.  I understand that whenever a photo-shoot is planned in advance and is under the control of the DCJS, I have the option to step out of the photograph if I so desire.
